Complete the SOAP Presentation Debrief Qualtrics form with a faculty member during
your debrief session.

Recommend reviewing and choosing a chronic renal/GU/endo chronic issue to review if
possible (Example: DM, Hypothyroid, Incontinence, Recurrent UTI)
Review (recommend playing at 1.5-2X speed)
Review with minor additions to assist with chronic disease / primary care note documentation

Recorders must complete your patient's note by 5:00 PM the day after your patient
encounter and sign/submit your note to Clinical Experience Preceptor and partner in
a [secure] email. If this is not completed on time, you will lose professionalism
points.
Once your note is sent to your Clinical Experience Preceptor, Recorder fill out  to receive documentation feedback.

COMPLETE YOUR BLS MODULE OVER BREAK!!!!! Bring printed or electronic copy of your certificate with you in July.

â–ª If you do NOT complete your module and bring your certificate, you will have to re-do
your practical experience with the AHA center (2ndfloor CJ) at a cost of $70 (paid
by you).
You will complete your BLS checkoff on your assigned clinic day. You MUST sign up using the link associated with your clinic date before midnight on the day of your clinic.
